Please fill out the form online and print. Sign forms and mail them to Wheeler Band Booster Club | 2100 Roswell Rd., Suite 200C – PMB 1208 | Marietta, GA 30062. Please complete and return the forms as soon as possible. If you are unable to obtain a notary for your signature, mail the form in anyway. We will have notaries available at the parent meetings on July 13th and 14th.
